• The proprietors and their employees will take reasonable care, they cannot
be held responsible for the death, injury, loss, or damage of any kind
whatsoever that may occur to any animal left in their care.
• I give the proprietors full authority to seek veterinary attention at my expense
should it be deemed necessary.
• My dog/s are fully vaccinated against Parvovirus, Hepatitis, Distemper,
Parainfluenza (C4).
• My cat/s are fully vaccinated against Feline Enteritis, Feline Rhinotracheitus
and feline Calicivirus (F3).

• I have provided all information concerning special problems or behaviours of
my animal(s).
• I understand that the proprietors accept no responsibilities for collars, leads,
bedding etc left at the kennels/cattery.
• I agree that in the event of arrears or after sue notice in writing, the proprietors
shall have the right to dispose of the animal(s) and this shall not release me
from the liability for any unpaid fees or charges.
• I agree that if my animal(s) cause any damage to the Kennels/Cattery I will
pay for the necessary repairs.
